LIABILITY WAIVER AND RELEASE OF CLAIMS

Next Level Fitness PT ("Company") requires all participants to sign this Liability Waiver and Release before participating in any fitness training sessions, consultations, or physical activities on or off premises.

1. Acknowledgment of Risk
I understand that physical exercise involves inherent risks including, but not limited to, muscle soreness, strains, sprains, cardiovascular events, and other physical injuries. I acknowledge that these risks exist and voluntarily choose to participate.

2. Medical Clearance
I represent that I am physically fit and have no medical conditions that would prevent me from participating in a fitness program. I agree to consult a physician before beginning any new exercise program and accept full responsibility for my own health and physical condition.

3. Release of Liability
In consideration of being allowed to participate in training sessions with Next Level Fitness PT, I hereby release, waive, discharge, and covenant not to sue the Company, its owners, trainers, staff, agents, and representatives from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained while participating in activities associated with Next Level Fitness PT.

4. Indemnification
I agree to indemnify and hold harmless Next Level Fitness PT from any loss, liability, damage, or cost they may incur due to my participation in fitness activities.

5. Photography & Media
I grant permission for Next Level Fitness PT to use any photographs or videos taken during training sessions for marketing and promotional purposes, unless I explicitly opt out in writing.

6. Cancellation Policy
I understand that cancellations made less than 24 hours before a scheduled session may be subject to a cancellation fee. No-shows will result in session credit deduction without refund.

7. Session Expiration Policy
I understand that purchased session packages expire 28 days (4 weeks) from the date of purchase. Unused sessions will be forfeited unless a manual extension is granted by the trainer or admin.

8. Governing Law
This waiver shall be governed by the laws of the applicable state. If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.

By signing below, I acknowledge that I have read and fully understand this waiver and agree to be bound by its terms.
